RELEASE AND WAIVER OF LIABILITY & CONSENT

This page is used for reserving your Basketball Training Session. I understand that by submitting this form I release the Better Basketball Boot Camp (BBC) from any liability for negligence that results in any injury to my child while attending any BBC camps. I agree to indemnify BBC, it's staff or volunteers and all other parties involved in the organization and administration of BBC from any such liability, and I represent that my child is medically fit to participate in rigorous physical activity.

In case of an emergency: I authorize BBC to secure medical treatment at no cost to BBC. BBC does not provide medical insurance. I also consent to the use of my child's photograph likeness for BBC promotional use.
